Put the paper on a darker table, shoot from above in daylight, and avoid the shadow of your hand and phone.
Which size should I choose?
For standard paper choose Letter or A4, and for receipts and cards choose Match the document, which keeps the real proportions.

Scan the pages one at a time as PDF, then combine them with Merge PDF.
